Health Monitoring
Our elderly health monitoring service helps families keep loved ones safe, healthy and independent at home. During daily or regular visits, our Carers pay close attention to things like appetite, mobility, memory and mood. They often notice subtle changes before they become more serious. It’s a practical and reassuring way to help prevent problems and make sure the right support is in place when it’s needed.

We can help with:
Checking vital signs like blood pressure, pulse, temperature and oxygen levels.
Supporting correct medication use and maintaining MAR (Medication Administration Record) charts.
Observing changes in mood, memory or behaviour to identify early signs of things like dementia or depression.
Tracking mobility and balance, and recording any falls or near-falls to keep loved ones safe.
Monitoring appetite, hydration and weight to prevent malnutrition or dehydration.
Managing specific conditions such as COPD, heart disease, diabetes or Parkinson’s.
Keeping detailed daily notes and alerting GPs, district nurses or family if health changes.
Using technology, like wearable devices or telehealth systems, for round-the-clock reassurance.

How our health monitoring service works
Health monitoring at home means regularly checking a person’s physical and mental wellbeing so they can stay safe and independent. Our trained carers carry out wellbeing checks and record accurate health information to spot changes early. If something unusual is detected, we act straight away, contacting family, GPs or emergency help if needed. This personalised care helps older adults stay well for longer and gives families reassurance that their loved one is safe and well at home.
