How our medication administration service works

We begin with a thorough assessment of your loved one’s individual needs, their health conditions, current medications, any changes from GP or hospital, and their home environment. From this assessment, we create a personalised medication plan that fits within their overall support.
Our trained caregivers then support your loved one by organising the medications (e.g. dosset boxes, blister packs or original packaging as appropriate), setting up reminders, administering medication at the right times, and logging every dose.
Your management team will also liaise with you, your GP and pharmacy to flag any prescription changes, missed or double doses, and monitor for side-effects or signs of medication issues (like confusion, fatigue, or dizziness).
We build medication administration into the full suite of elderly care services, ensuring everything works together to support daily wellbeing.
As your loved one’s needs change, so does the support. If new medications, if there are swallowing difficulties, or changes in cognition, we review and update the plan.
You’ll receive regular updates, visits and ongoing communication, so you can be confident your loved one is safe, supported and well cared for at home.
